The new Madcap is an Epic Outfit in Fortnite: Battle Royale, that can be purchased in the Item Shop for 1,500 V-Bucks. Madcap was first released in Chapter 3: Season 1 and is part of the Fungus King Set. The skin can be obtained with V-Bucks from the Item Shop in the usual manner.
Madcap is bundled with the Bello Bag Back Bling. This outfit had been leaked before its release in the game, and it is currently available to purchase in the near future from the item shop. The skin has two selectable styles to choose from: the Default and the Glow version.
The new skin is a part of the epic outfits in Fortnite: Battle Royale, which is not very different than the rest of the collection. Madcap was officially added into the files in Chapter 2: Season 3, although encrypted.
