In 2011, Mysterio gained his first victory at Extreme Rules by defeating Cody Rhodes in a Falls Count Anywhere match. In 2020, he made his fourth appearance at the pay-per-view where he participated in one of the most deadly matches in WWE history. He was defeated by Seth Rollins in an Eye for an eye match which marked his third loss at the event.
